Fungal external otitis - epidemiological, clinico-mycologic features and treatment implications PDF Print

Objectives: The aim of this study was to determine epidemiological, clinicomycologic features, predisposing factors and treatment outcomes of fungal otitis externa.
Material and method: We performed a retrospective review of 116 patients with diagnosis of fungal otitis externa treated from 2002 to 2006 in ENT Clinic of Military Hospital Iasi.
Results: The distribution of patients by address and sex was higher in townspeople (61,20%) and males (59,47%). The most common presenting complaints were aural fullness and pruritus (60,34%). The treatment consisted in mechanical debridement and topical antiseptic agents in patients without prior otic or medical histories and a topical antifungal agent (Clotrimazol 1%, Naftifinum 1%) in addition in patients with prior otologic procedures or related chronic systemic pathology. The duration of treatment was longer in the last group. Patients with chronic nasal respiratory insufficiency had been operated on with different rhinological techniques.
Discussion and conclusions: Due to their non specific symptoms, fungal external otitis require rigorous local and general evaluation. The predisposing factors for the development of fungal external otitis should be avoided or treated. An accurate mechanical debridement completed with topical antiseptic and antifungal agents is generally effective. For a complete recovery the treatment must be directed towards restoring local and general immunologic,
metabolic and physiologic status.
